| In recognition of an individual practitioner who is Involved, Motivated and Valuable to his/her profession. This Award shall be presented to an individual in the New York Downstate association for Respiratory Therapists who is dedicated and upholds the aims and goals of both NYDART and the AARC. The individual shall demonstrate. 1. A contribution to the science, technology, ethics and art of Respiratory Care. 2. Excellence in the practice of Respiratory Care as a clinician, educator or administrator. 3. Contribution to the growth of the field as evidenced by his/her clinical practice and participation in professional activities. SPECIFIC CRITERIA: The nominee must: 1. Be an active member of the AARC. 2. Complete at least five years of service to the profession of Respiratory Care. 3. Demonstrate significant achievement in one more of the following area in the preceding two years. a. professional practice (service to the community, job related activities, professional presentations, offices held in professional societies) b. academic achievement (level of education, professional credential, scholastic record) c. professional service to NYDART (committee work, offices held, participation in sponsored events) NOMINATIONS: 1. Nominations may come from any active member of the AARC. 2. The nomination must be submitted to the NYDART Awards Committee Chairman no later than August 1, 2010. 3. Based on the nominations received, the President of NYDART will request that the prospective candidates submit a current curriculum vitae and the names of two additional references to the Board on or before September 1, 2010. 4. The award will be given at the annual NYDART Symposium September, 2010. 5.
